University of Ilorin Hospital (UITH) Admissions Form 2023

It began operations in July 1980 and began operating as a temporary facility. It was the Ilorin General Hospital and Maternity Hospital which was then owned by the Kwara State Government.

The permanent location of the hospital was declared open by His Excellency the President of the Federal Republic of Nigeria, Chief Olusegun Obasanjo, in May 2007, while the full move took place in April 2010.

Applications are invited from qualified applicants for positions not listed here:

  • MBBS; NYSC Discharge or Exemption Certificate
  • Current registration/professional license with the Medical and Dental Council of Nigeria.
  • Experience from a tertiary institution and post-NYSC will be given special consideration.
  • A referee’s report containing at least one item from the applicant’s former medical professor.
  • Hold a master certificate in the appropriate specialty.

Application process

  • The application form must be completed from the University of Ilorin Teaching Hospital website ( or the link above. Five thousand naira (N5,000.00) non-refundable fee is payable into the University of Ilorin Teaching Hospital account with CBN through the electronic payment gateway CBN REMITA.
  • All data and scanned uploads (e.g. certificates, receipts, Remita, etc.) in form fields must be correctly specified as required for processing your form.
  • Payment must be made before you can proceed to the form page.
  • How to pay: Login to
  • Click FGN and State TSA Payment.
  • Select and click FGN Federal Government of Nigeria.
  • Print University of Ilorin Teaching Hospital that you want to pay
  • Select General Purpose UITH. on the service name/purpose

  • After paying through the Remita platform, you can complete your registration by filling in your information online. Consultation form With the Remita Retrieval Reference Number (RRR) generated with the payment.
  • Click to apply now. You will then be redirected to an interface where you will be asked to enter a valid personal email address.
  • A confirmation link will be sent to the email you provided during email registration.
  • Log into your email account by opening a new tab in the same browser as the one you already opened.
  • Click on the link sent to your email at noreply@uithilorin
  • A mentor application form will be available for you to complete and submit.
  • Select the appropriate position you want to apply for in the drop-down menu.
  • Fill in information in other fields. in the form as desired
  • After submitting, wait 2 seconds and you will be directed to print or download the completed form.
  • NOTE: Please do not bring printouts to UITH.
